The applicant is advised to make an appointment online before coming to the consular office, Royal Thai Consulate -General,Chicago or the Mobile Consular Units, as biological data must be collected (photo, retinal scan, and fingerprints). This is because you will receive a confirmation email instantly from the automated response system upon successfully scheduling an appointment. In the email, the date and time of your selected appointment will be specified, along with a checklist of the documents you need to bring on the day of your service request. This is to facilitate your document preparation and avoid long waiting times for service or even the possibility of not being able to obtain a passport. Additionally, the officers will provide service to individuals with advance appointments before serving those without appointments.

When the appointment is confirmed and you have the date and time that you can come to submit your application, please prepare the documents and arrive at the address below.

1. Individuals who wish to apply for a passport must come to the Consular office in person as there will be a biometric data collection, such as fingerprinting, photography, and iris scanning. 

2. Avoid wearing colored contact lenses on the day of service.

3. Passports have a validity of 5 or 10 years and cannot be renewed within the same book. If your previous passport has expired, you must apply for a new one.

4. The name on the new passport will be based on the Thai national registration database, which may not necessarily match the name on the previous passport.

5. If you wish to have your last name in the passport changed due to marriage or divorce, you must update your last name in the Thai national registration system first (update at the district or sub-district where your name is registered).

6. Individuals born in the United States are eligible to apply for a Thai passport only once for the purpose of returning to Thailand and registering their residence and obtaining a national ID card. If you receive your first passport and do not use it to return to Thailand for these purposes, the Ministry will not approve the issuance of a second passport until you register your residence and provide a 13-digit national ID card to the officers.

7. Individuals aged over 20 years who have previously held a Thai passport and have not obtained a national ID card will not be able to apply for a new passport. They must return to Thailand to apply for their first ID card at the district office or sub-district office. Please consult with a passport officer to verify the eligibility in applying for an Emergency Travel Document (ETD) instead. 

Note: If you have an urgent need to travel, such as a family emergency or a court summons requiring immediate return to Thailand, you can apply for an Emergency Travel Document (ETD) or an Emergency Passport (EMP). It's essential to follow the instructions and contact the Royal Thai Consulate-General in your area promptly to expedite the process and address your urgent travel needs.

6. Thai House Registration indicating the applicant's first name, last name, and 13-digit identification number (only if there has been a change of address previousely)

2. A photocopy of the current passport (only the personal data page)

3. An original of a valid Thai ID card

4. A Passport Lost form ( will be filled out at Royal Thai Consulate)

5. Other documents to confirm your place of birth, such as a Thai birth certificate (only if you do not have a copy of your passport's personal data page) 

6. A Thai House Registration indicating the applicant's first name, last name, and 13-digit identification number (only if there has been a change of address previousely)

If you have legally changed your last name in the civil registration system, and you need to update this change in your passport. Please ensure that the change of your last name has been legally registered in the civil registration system of your country. This typically involves obtaining a legal document, such as a marriage certificate, court order, or other official record, indicating the last name change. 

You will need to provide the legal documentation supporting your last name change, along with other required documents as follows: 

1. A Passport Application Form (which can be filled out in the Passport Appointment system) 

2. An original of a valid Thai ID card

3. An original of a current passport

4. A Thai House Registration indicating the applicant's first name, last name, and 13-digit identification number (only if there has been a change of address previousely)

5. A Legal name change certificate

6. Passports fee paid by Money Order or Cashier’s Check made payable to "Royal Thai Consulate - General, Chicago" (cash, personal checks, and credit cards are NOT accepted)
