Notification of death and the issuance of a death certificate

 

(1) Regulations for reporting a death

  1. The person notifying the death should be a family member or have a familial relationship with the deceased, such as a child, parent, spouse, sibling, or relative.
  2. The deceased person must have passed away within the jurisdiction of the Consulate General in Chicago

 

(2) Supporting documents for submitting a death notification application include:

  1. A death certificate issued by the local government agency (Death Certificate from the State), which depends on the state of the deceased, and must be authenticated by the Secretary of State of the issuing state. For other states, please  Click here to find more information
  2. The Thai passport and the Thai national ID card of the deceased person.
  3. Copy of the Thai passport or the Thai national ID card of the person notifying the death.

 

There are two forms for filling out the application for a Thai death certificate:

  1. Filling out the legal death notification form
    Fill in detailed information about the deceased, including clearly stating the cause of death in Thai.
  2. Filling out the Thai death certificate form
    Fill in the information on the death certificate form in Thai, making sure all fields are completed in detail. Sign the form in the 'Notified by' section, ensuring the signature and surname are written in Thai only.

 

To report a death by mail, send the completed death registration application form and the Thai death certificate form with signatures, along with a Notary Public Form that has been notarized by a local government representative (Certified by Notary Public). Also include copies of documents 2.2 and 2.3 (document 2.1 should be the original). Send all documents to the Royal Thai Consulate General, Chicago and include a self-addressed envelope with postage (Priority Mail) so that the Consulate General can return the documents and the Thai death certificate to the notifier by mail.

 

Once the Consulate General has received all the correct and complete documents, a death certificate (Form Thor.Ror. 11, Part 1) will be issued to the notifier. This death certificate must be presented to the local registrar in Thailand where the deceased is registered in the household registration to have the name removed from the household register.
