Birth Registration and Issuance of Thai Birth Certificate
- The person registering the birth must be the father or mother, or a person who holds Thai nationality. The application can be submitted by mail.
- Eligibility of the child: The child must be the offspring of a father or mother who holds Thai nationality.
- The child must be born within the consular jurisdiction of the Royal Thai Consulate General in Chicago.
- Supporting documents for birth registration include:
4.1 Thai passports of the father and mother, valid for at least 6 months from the expiration date
4.2 - Birth certificate issued by a local government agency (original birth certificate from the State)
- The State birth certificate must be authenticated by the Secretary of State before submission.

- Example of an authentication certificate
4.3 Marriage certificate (marriage registration) which may be issued by either a local government agency or a Thai government agency
4.4 Birth weight certificate issued by the hospital (hospital birth certificate)
4.5 A recent 2"x2" photograph of the child (1 copy)
4.6 Other identification documents of the father and mother, such as a copy of the Thai national ID card or household registration
4.7 A copy of the local identification card (Driver's License or State ID) of the father and mother (if available)
Thai Birth Certificate Request Form
There are two types of forms for requesting a Thai birth certificate:
- Legal Birth Registration Form (in Thai)
This form requires detailed information about the child, father, mother, and the person registering the birth. All information must be filled out in Thai. If the child has a middle name on the birth certificate issued by a local government agency (State birth certificate), be sure to include the middle name with the correct Thai spelling in the Legal Birth Registration Form.

Complete all details in the Thai Birth Certificate Form attached, and sign it in Thai.
- Mail-in Birth Registration
If registering by mail, send the completed Legal Birth Registration Form, the filled-out Thai Birth Certificate Form with the signatory's signature (signed according to the name on the Thai ID card), along with a Notary Public certified signature form. Also include the document from section 4.2 authenticated by the Secretary of State (Authenticated by Secretary of State) (more information Click here), and other supporting documents certified as true copies by the person registering the birth. Mail these to the Royal Thai Consulate General. There is no need to send original documents, except for item 4.2. Additionally, include a self-addressed stamped envelope with a Priority Mail, so the Consulate can return the documents and the Thai birth certificate by mail.

Important Information
- The birth certificate is an important document necessary for submitting a request to register a child's name in the household register with the local registrar. This is required to obtain a person’s entry and a 13-digit ID number in the national population database. It is also related to the individual's rights and responsibilities, such as voting and applying for a passport. Therefore, the recipient of the birth certificate should check the details to ensure the information is recorded correctly. If any information is incorrect, please notify the authorities to correct it immediately.
- In the case of a Thai national born abroad who wishes to add their name to the temporary household register to obtain a 13-digit ID number, the child’s parents, if they are not of legal age, may issue a letter of authorization for a representative to handle the matter on their behalf.
- If the birth certificate is lost, report it to the local police, and you can request a certified copy of the birth certificate from the Royal Thai Consulate General or the Central Civil Registration Office, Department of Provincial Administration (Wang Chaiya, Nang Lerng), Nakhon Sawan Road, Dusit District, Bangkok.
